How to resolve
  1. Acknowledge within MTTA (on-call).
  2. Investigate the flagged audit_log row — see runbook.
  3. Record a chain_break_recorded entry via appendAuditEntry — never UPDATE / DELETE audit rows.
  4. Invalidate old lifecycle PDFs (regenerate + redistribute).
  5. Resolve & Clear (super_admin) — paste postmortem note, quarantine lifts.
  6. Verify — re-run chain-verify hook, drop a fresh Evidence Bundle on /verify, confirm PASS.
